Diverticulitis Flare 09/03/2022

 I spent Saturday in debilitating pain, and finally on Sunday realized that it was necessary to get medicine.  Went to Quick Care.  The doctor wasn't certain of the diagnosis.  He wanted to make sure it wasn't something more serious.  So he sent me to the emergency room for a cat scan.  They confirmed diverticulitis.  They prescribed Cefpodoxime 100 mg 2x daily for 7 days.  They also prescribed Amoxicillin-Clav for and uti.  They also recommended a followup with a GI.  I'm conflicted about seeing another GI because I think it's excessive.  I'm current on colonoscopies and I know the diagnosis.

I spent Monday resting, and today, Wednesday, is the first day that I've started feeling better.

IBS Diagnosis and Low Fodmap Update

Last Wednesday, February 3 2021, I saw a new gastroenterologist who confirmed the primary doctor's diagnosis of IBS.  He agreed that if Low FODMAP was helping, that I should stick with it.  He talked with me about the prescription Dicyclomine I had been given.  It was a higher dosage because I was taking it during acute symptoms.  He recommends a lower dose, taken regularly to prevent symptoms.  I haven't started it yet, because the Low FODMAP diet is working so well, but I am considering it.  I have downloaded the Monash University Low FODMAP app, and I am regularly tracking foods and symptoms.  I am beginning to feel much better.  My main question now is, how does this relate to diverticulitis?  According to the gastroenterologist, those who have repeat cases of diverticulitis are at greater risk for future cases.

Learning about the Low FODMAP Diet

 As I begin to dig into the Low FODMAP diet, and cross-reference it with the SCD diet, I am learning a great deal.  Here I would like to list links to information, as well as lists of foods that I am able to tolerate.

I seem to tolerate hard-boiled eggs and cheddar cheese curd.  I tolerate broccoli.  I made a low FODMAP smoothie out of pineapple, blueberries, bananas, and avocado that I seemed to tolerate well.      

I have read that V-8 is low FODMAP, and intend to try some today.

For ease of reference, here are the SCD legal foods.  Up until this flare, the SCD Diet was applied whenever symptoms began, and it helped, but more recently it hasn't been enough:

Even though today is January 24, 2021, I intend to continue to add to this list, documenting the foods that do not cause pain, and links to helpful information.

The Flare Worsens, and Low FODMAP seems to be helping

I had been on the Specific Carbohydrate Diet for the last two weeks since that has worked in the past.  Last week, on January the 17th, 2021 I was beginning to feel better.  However, as the week progressed, I began to experience diarrhea, constipation, painful bloating, and severe abdominal cramps.  One night I thought about something I had read about someone making an incision in a cow to let out bloat, and I briefly thought I would prefer that to the continued pain.  

I mostly persisted with the SCD diet, and couldn't figure out why I was experiencing severe abdominal pain after meals.

Yesterday, Saturday the 23rd, 2 1/2 weeks after the beginning of this flare, I was in extreme abdominal pain with constipation.  I spent the day in bed.  I had started taking Alka Seltzer the night before, and it was helping with the bloating and cramps, but only for four hours, and as soon as it wore off the pain would once again become intolerable.  I had to get up in the middle of the night and take more Alka Seltzer just so that I could sleep.  The Alka Seltzer label says that you should only have 8 tablets in a day, but each dosage is 2 tablets, and keeping the pain at bay would mean exceeding the dosage, so I began to look for other alternatives.

I began taking, Dr. Tobias Oregano & Caprylic Acid Blend - with Probiotics - Supports Flora Balance

I also took Mira Lax, (which did nothing,) and CVS Health Intestinal Defense Peppermint Oil softgels.  I'm not sure if the peppermint oil helped with the cramping, but it was pleasant.

However, I found an old bottle of  Dicyclomine which had expired in 2017, and took a tablet out of sheer desperation.  It helped with the cramping.  I only needed to take 2 tablets, the second one  8 hours after the first one, and was able to sleep through Saturday night.

I also began to see a pattern:  I was experiencing severe abdominal pain after eating foods that were Specific Carbohydrate Legal but high FODMAP.  For example, honey is SCD legal but high FODMAP.  Nuts are SCD legal, but cashews and high FODMAP.  

For now, I am going to eliminate all foods that are not allowed on both diets.  As I begin to feel better, I will add healthy foods that are not SCD legal but are still low FODMAP, and then gradually begin to add high FODMAP foods.

It might seem odd that I didn't visit my doctor, and there are several reasons for this.  

First of all, we are in the middle of a pandemic, and if I can manage my own symptoms without going out to public health facilities, that's probably safer.  I can always see a doctor if I can't get a handle on the symptoms.  

Also, I work during the week, and my doctor's office is closed on the weekends.  If I see a doctor on the weekend it will have to be a quick care doctor, and both my primary physician and my gastroenterologist have expressed that it's difficult to base a diagnosis on hearsay from a quick care doctor without seeing the symptoms themselves.  Of course, this is frustrating for me, because I can clearly tell them what the symptoms were even though they have passed.  But it means that if I am truly going to get a handle on this, I will have to organize a day off so that I can see my doctor while I still have active symptoms.  Since days off are normally scheduled a week out, and I have no way of knowing whether I will still have active symptoms in a week, it's better for me to figure out how to avoid pain by avoiding certain foods.  I can always call in sick in an emergency, but my employer, just like most employers, will start to consider disciplinary action if that occurs too often, so I will avoid calling in sick unless it really is impossible for me to work.

And lastly, with the exception of a quick care doctor prescribing Dicyclomine, I am under the impression that there is little that doctors can do about this.  It might be better for my condition using one of my paid time off days to rest, rather than going out to a doctor's office.

However, if I want more Dicyclomine, which I do, it would be best to see my physician while experiencing active symptoms.  So there it is.

Update:  After being so sick on Saturday, January 23, I ate strictly low FODMAP and FODMAP free foods on Sunday.  I felt much better,.  Today, Monday, January 25, 2021, I at low FODMAP again, and my intestinal function is almost back to normal.   By normal, I mean more "normal" than it has been in many months.  I haven't taken any OTCs or supplements at all today.  I plan to stay on the low FODMAP diet for six weeks and then regroup from there.  Six weeks from today is Monday, March 8, 2021.

Second Flare Up In Less Than One Month

I have been taking some pretty strong probiotics daily, but I have not been keeping my intake of long-chain carbohydrates down, because the symptoms abated.  It's been the holidays, and I have decided that for the holiday season I would eat whatever I wanted to.  My diet has probably not been optimal, especially for someone with an apparent chronic condition.  I really thought the probiotics would protect me.

About a week ago, on January 6, I began to feel very fatigued and vaguely nauseous.  I chalked it up to political events happening on that day.  Right-wing extremists stormed the US capitol at Trump's bidding, and I was horrified.  I thought that might be why I was feeling off.  The feeling persisted.  By Sunday night, I was so exhausted that I nearly fell asleep in my chair even though it was early evening.  This had never happened before.  I was a little worried about having persistent mild nausea, but not so much that it affected anything.   I do not recall my earlier symptoms including fatigue and nausea.

About 4 days later, I woke up at 3:00 a.m. Monday morning with sharp abdominal pains, leading to severe diarrhea.  This persisted until about 8:30 in the morning.  I could not get any rest.  Finally, at around 9:00 a.m., it began to ease up and I was able to sleep.  I drifted in and out of sleep that day.  I ate applesauce and hard-boiled eggs.  Ehren brought me a fried egg for dinner on a bed of cucumbers, tomatoes, and topped with avocado.  I tolerated all of that well.  For the rest of the week, I limited my diet to Lara bars, boiled eggs, nuts, fruits, and vegetables.  Ehren fixed me some delicious chicken breast which was "breaded" with almond flour instead of bread crumbs.  

I have been steadily feeling better.  I'm still fatigued but not as much.  I still have a sensation like I have a belly full of broken glass, especially after meals, but it's getting better.  I haven't had any diarrhea.  

I really do think this is part of the same condition.  The reasons:

*I haven't left home due to the pandemic, so I haven't caught anything.

*No one else in the house was sick.

*The symptoms improved with a change in diet.

*I didn't eat anything strange or "off," just plenty of long-chain carbs.

I think that if it weren't for the probiotics it would have been worse.  So once again, I think it's important to eliminate long-chain carbs from my diet for now, and once I feel better, limit my intake.

And now, despite it being only 4:00 in the afternoon, I am going back to bed.  I plan to get maximum rest for the remainder of the week since I have split days off this week and I find it adds to my feeling of exhaustion.  

Update:  I didn't being to feel "normal" until Friday the 15th, nearly a week after the severe and unexpected diarrhea symptoms began.  Today is a week later, Sunday, and I am beginning to feel like I can think about something other than being sick again.  However, I feel anxious and apprehensive because I am afraid that it will come back and bring more pain.
